| Gender | State your gender. (M F) |
| Student Number | Submit the 6 digit student number given to you in your enrollment package |
| Email Address | Give us the email address what you use most frequently. Enter none@all if you do not have one. |
| Area Code | Provide us with your area code. If you are not in North America, give the country code as well. |
| Phone Number | Enter your phone number. If there are fewer than 7 digits enter the letter x until there are. |
| High School | Provide us with the name of your high school. |
| H.S. Province | State the province/territory/state where your High School is located. |
| H.S. Country | Provide the country where you last attended High School, na if you did not attend any grades. |
| Frosh Week | Select the checkbox if you are planning to attend Carleton Engineering/Industrial Design/Architecture Frosh Week. The cost can be found in the Frosh Zone. |
| Frosh Kit | Select if you wish to purchase a Frosh Kit from EngFrosh '99. The cost can be found in the Frosh Zone. |
| Bungee Day | Select if you would like to go bungee jumping at some organized time during FroshWeek. The cost can be found in the Frosh Zone. |
| Credit Card # | Enter the full serial number from the credit card you wish to use. |
| Expiry Date | Type in the expiry date of your credit card. |
| Street and Appartment Address | Enter the street number, name and any apartment information necessary for us to mail you information. |
| City | Provide the city of your mailing address. |
| Province | Type in the province/territory/state of your mailing address. |
| Country | Type in the country of your mailing address. |
| Comments? Concerns? Requests? | Give us any feedback, ideas or worries you may have about any aspect of FroshWeek. This may not get you any immediate response. You may also like to try to contact us by phone or email. |
| Submit to Server | Click on this button when you are done entering the data. If anything went wrong, a window will appear and ask you to fix any mistakes. Nothing can be submitted until this window is closed properly by clicking on the Close Alert Window button. |
